Need help understanding your pathway?

Date Updated: 2026-08-23

If you’re a Specialist International Medical Graduates (SIMG) or a Doctor who achieved a Fellowship in General Practice overseas and are looking to pursue that Specialist area in Australia, then you can pursue the Specialist Pathway.

You mainly have two options to choose from, specifically the Expedited Specialist Pathway and the College-led Specialist Pathway (Specialist Recognition). While both lead you to Specialist AHPRA Registration in Australia, the journeys to get there are a bit different.

In this blog, we’ll walk you through the key differences—like eligibility and the steps involved—so you can figure out which pathway works best for you.

Key Differences

Here are the main differences between the two pathways:

  • Management
    • Expedited Specialist Pathway is directly managed by the Australian Health Practitioner Regulation Agency (AHPRA), along with the Medical Board of Australia.
    • College-led Specialist Pathway may be managed by the Australian College of Rural and Remote Medicine (ACRRM), or the Royal Australian College of General Practitioners (RACGP), depending on the college of your choice.

  • Fellowship
    • Expedited Specialist Pathway does not lead to holding a Fellowship with either college. This would need to be pursued separately with the college.
      • RACGP has opened a new Fellowship option for Doctors who completed the Expedited Pathway and received their unconditional specialist registration. See it here.
    • College-led Specialist Pathway requires Fellowship for the college you have chosen as part of the pathway.

  • Assessments
    • Expedited Specialist Pathway does not require you to go through a comparability assessment or, since it’s optional, Fellowship exams.
    • College-led Specialist Pathway requires you to go through a comparability assessment, and depending on the college and your qualifications, may require you to pass the Fellowship exams.

  • Time & Fees
    • Expedited Specialist Pathway is a faster way to get Specialist Registration and has less fees since you are not required to pay any membership fees, comparability assessment fees, and program fees.
    • College-led Specialist Pathway has more requirements to complete and has both membership, comparability assessment fees and program fees, specifically:
      • ACRRM
        • Program Time:
          • Substantially Comparable: 3 – 12 months
          • Partially Comparable: 6 months – 24 months
        • Program Fee: $2,940 annually
        • Comparability Assessment Application Fee: $800
        • Membership Fee: $495
      • RACGP
        • Program Time
          • Substantially Comparable – FT: 12 months & PT: 24 months
          • Partially Comparable – FT: 24 months & PT: 48 months
        • Program Fee: $12,525
        • Comparability Assessment Application Fee: $605
        • Membership Fee: $661

Let’s dive in and learn more about each pathway together!
