This is to confirm that the above mentioned Doctor has been offered and accepted a contract with our practice.
The Doctor has met their Medicare legislative requirements as demonstrated in the attached documentation.
The Doctor will be providing greatly needed General Practitioner services to our community and therefore require a Medicare Provider Number to allow Medicare rebatable services.
