What paperwork do I need to complete for a Non VR Doctor under Supervision?

For PESCI:
If your doctor is applying for an RACGP PESCI, you will need to prepare:
- RACGP CV
- Position Description with the PESCI Position description template
- Signed PESCI Applicant declaration form
- SPPA-30
- Proof of a pass in the AMC CAT MCQ Examination
- *If applicable:
- Evidence of name change
- For those applying for provisional registration, evidence of completed AMC clinical component or eligibility for the Competent Authority Pathway is required.
- For original documents that are not in English, you must provide certified official and endorsed English translations of these documents in addition to the originals.
If your doctor is applying for an ACRRM PESCI, you will need to prepare:
- ACRRM CV
- Position Description with the College-formatted template
- Signed PESCI Candidate Agreement
- A certified copy of your passport
- *If applicable:
- For those applying for Limited Registration – Post Graduate Training and Supervised Practice, a Training Plan is needed for the application submission.
If your doctor is applying for an IME PESCI, you will need to prepare:
- IME CV
- Position Description with IME’s template
- Evidence of a medical qualification as listed on International Medical Education Directory (IMED)
- Certified copy of the applicant’s passport
- Completed AMC MCQ or be eligible for provisional registration through the Competent Authority Pathway
- *If applicable:
- For those applying for Limited Registration – Post Graduate Training and Supervised Practice, a Training Plan is needed for the application submission.
For AHPRA:
If your doctor is applying for Limited Registration Post Graduate Training they will need to submit:
- AHPRA CV
- Position Description
- SPPA-30 form
- Application Form
- Training Plan
- AHPRA Letter
- ALPS-30
- Supporting documents, if applicable
If you also have been absent from practice for more than 3 years, you would need to submit a Professional Development and Re-entry to Practice.
If your doctor is applying for Limited Registration Area of Need they will need to submit:
- AHPRA CV
- Position Description
- SPPA-30
- Application Form
- Area of Need Certificate
- AHPRA Letter
- AANG-30
If you also have been absent from practice for more than 3 years, you would need to submit a Professional Development and Re-entry to Practice.
If your doctor is applying for Provisional Registration they will need to submit:
- AHPRA CV
- Position Description
- SPPA-30
- Application Form
- AHPRA Letter
- APRI-30
If you also have been absent from practice for more than 3 years, you would need to submit a Professional Development and Re-entry to Practice.
As the Practice Manager, you will be responsible for preparing and submitting the Area of Need Certificate application on behalf of the General Practitioner. You can also fill out the Position Description, SPPA-30, Training Plan and sections of the AHPRA Application form for the doctor or provide them with the relevant details.
After AHPRA
Once your doctor’s AHPRA application has been approved you will need to apply for a Medicare Provider Number and potentially a 3GA program depending on their restrictions.
If your doctor is a Permanent Resident or Citizen, they will need to apply for a 3GA program.
To do this, you will need to prepare a HW019 form and a Medicare letter of support and if applicable the relevant paperwork for a 3GA program.
For 3GA Programs
ACRRM Fellowship Programs (AGPT, IP, and RGTS)
- Recent colour photo
- Proof of photo identification (licence or passport)
- Evidence of current residency status
- *If applicable:
- Evidence of name change
- Pathway to Medical Registration (AMC Certificate or Competent Authority Pathway 12-month supervision assessment letter)
- Evidence of active Australian permanent residency application or intention to apply for permanent residency
- Temporary Visa Holders: Temporary Skills Shortage (TSS) 482, 457, 491 and 494
RACGP FSP
- Personal details
- General practice experience
- Current Australian Medical Registration
- Training site name & location
- Letter of support within application form (each practice applied for)
- Supervisor details
- Supporting documents (primary medical degree, & accreditation status of current or intended practice
RACGP AGPT
- Mandatory personal details and work-related information fields
- Mandatory supporting documentation
- Referee details
- Training region and pathway preferences
RVTS
- Curriculum Vitae
- A letter of support from current practice, dated and signed by the Practice Principal, Senior Medical Director, CEO, Director, or Practice Manager
- A passport-sized photo
- Evidence of Australian or New Zealand citizenship or Australian permanent or temporary residency
- Evidence of name change if applicant’s name appears differently in supporting documents
- *If applicable:
- Sworn Statutory Declaration of intention to resign from AGPT or ACRRM Independent pathway or RACGP Practice Experience pathway if selected into the RVTS program
- If working in a solo doctor town, a statement from the relevant Rural Workforce Agency or Primary Health Network verifying that it is a solo doctor town
- Evidence of pass in AMC Part 1 exam or certified copy of Advanced Standing Certificate Evidence of the date of first medical registration in an Australian state or territory (eg certificate or letter from AHPRA) (including New Zealand trained doctors)
- Evidence of Level 3 or 4 Supervision
- Evidence of Aboriginality
