I am ready to change my supervision level, what do I need to do..?
If you are ready to move supervision levels i.e. from Level 1 to Level 2, or Level 2 to Level 3, you will need to submit some more documents to AHPRA.
These include the ACCL-30 Form which is an application for a change in circumstances.
You will need to prepare a WRIG-30 which is a Work Performance Report (these should be prepared quarterly or whenever AHPRA requests it).
Don’t forget to update your Supervision Plan (SPPA-30) to reflect your change in supervision levels.
